Registered Nurse – Home Health Location: Mesa, Arizona Schedule: 40 hours per week. Hours worked between 7:00 AM – 6:00 PM. Coverage area: Approximately 60‑mile radius from the agency. Mileage reimbursement provided between patient visits (first patient to last patient). Responsibilities Conduct daily patient visits in home and hospice settings Complete patient admissions and assessments Develop, implement, and update patient care plans Coordinate care with physicians, caregivers, and interdisciplinary teams Provide patient and family education regarding care and treatment plans Respond to after‑hours patient needs and on‑call responsibilities during weekdays and weekends Maintain accurate and timely clinical documentation Ensure compliance with all regulatory and organizational standards Qualifications & Requirements Active Registered Nurse (RN) license Hospice and/or Home Health experience preferred Strong clinical assessment and patient care skills #J-18808-Ljbffr The Judge Group
