CCBHC Project Director

Team with the CCBHC Project Director in the implementation of a large, cutting-edge integrated behavioral health and primary care initiative at Catholic Charities' New Haven Behavioral Health Clinic. Provides care management to clients and supervises community health workers. Teams with Project Director to provide direction to Integrated Treatment Teams. Ensures strengths-based, person-centered, quality care is provided in a culturally and linguistically meaningful therapeutic environment.

General Duties and Responsibilities:

1. Responsible for teaming with the Project Director in implementing the primary and behavioral health care integration as part of the CCBHC Project at the Institute for the Hispanic Family Behavioral Health Clinic.

2. Co-leads project integrated care teams composed of clinical leaders, psychiatry, community health workers and case managers, as needed.

3. Provides integrated care management to a patient panel of CCBHC clients, ensuring linkage to on-site and community primary and community specialty care. Provides medical monitoring of patients with chronic medical conditions. Ensures client data collection and EMR entry on blood pressure, body mass index, lipids, glucose, and tobacco use on a scheduled basis.

4. Collaborates with a local FQHC, which is co-located with the CCBHC; provides patient information to FQHC clinic provider, as needed, with client consent.

5. Conducts and reviews health care screenings and assessments. Ensures client linkage to primary and specialty care as needed, working in collaboration with Intensive Community Health Workers (ICHWs).

6. Supervises community-based, integrated care staff. Delegates assignments and monitors effectiveness to ensure best practices within the program and compliance with operational plans and the CCBHC scope.

7. Works with external collaborators, including the CT Department of Mental Health and Addiction Services, local Federally Qualified Health Centers, primary and specialty care providers, evaluators and others to manage patient care and project reporting.

8. Other duties as assigned.

Qualifications:

Bachelor's Degree in Nursing/RN license with 1-2 years of related experience in behavioral health or integrated care, or Licensed Practical Nurse (LPN) with 2-4 years of related experience including staff supervision.

Strong belief and commitment to integrated care models; Knowledge, experience and sensitivity to working with clients with behavioral health, addiction and co-occurring disorder.

Exercises good judgment and decision-making. Ability to deal with sensitive issues with diplomacy and discretion. Must be able to multitask with strong organization skills.

Knowledge and experience with evidence-based practices in behavioral health and primary care.

Commitment to using data to drive decision making and rapid cycle change in programming.

Ability to use Microsoft Office Suite and Electronic Medical Record Software and Reporting.

Excellent interpersonal skills and able to communicate both verbally and in written form.

Commitment to cultural diversity and sensitivity.

Ability to maintain a professional demeanor under pressure and operate with a keen sense of urgency and commitment to quality.

Bilingual/bi-cultural skills (Spanish-speaking) preferred but not necessary.
