Develop individualized treatment plans to maximize independence and safety. Provide skilled therapeutic interventions in patients' homes. Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals. Complete timely electronic documentation while maintaining the highest standards of patient care. Qualifications: Graduate of an accredited Doctor of Physical Therapy (DPT) program. Licensed or license-eligible Physical Therapist in NJ. New graduates are strongly encouraged to apply! Excellent communication and interpersonal skills. Valid driver's license and reliable transportation.
