Were hiring an ECM Case Manager in San Jose California who wants a weekday schedule meaningful community impact and the opportunity to help high-risk Medi-Cal members overcome barriers to care.

Youll step into a community-based clinical role focused on outreach enrollment assessment individualized care planning advocacy and resource navigation. Youll work independently in the community while collaborating with multidisciplinary clinical teams and community partners.

What makes you a great fit
Bachelors degree in a healthcare or behavioral health-related field or equivalent training and experience
1 year of case management or related experience
Comfortable working independently and driving throughout the community
Valid California drivers license personal auto insurance and acceptable MVR
Organized resourceful culturally responsive and comfortable supporting complex needs
Trauma-informed experience and bilingual ability are welcomed

Compensation & Benefits
$25.00$28.50/hour
