Help patients perform therapeutic exercises and activities of daily living. Educate patients and families on care plans and progress. Maintain accurate documentation and patient records using EPIC Remote Client and Rover. Manage a caseload efficiently, seeing up to 6 patients per day. Required Experience / Certifications / Licensure Minimum of 1 year of skilled home health experience. BCLS certification. Reliable transportation (patient visits may extend up to 60 miles).
