Substance Use Case Manager & Care Coordinator
Outreach Development Corp.
Outreach is seeking a Case Manager to deliver care coordination for individuals with a history of substance use. You will work within the interdisciplinary treatment team, coordinating services and advocating for clients across agencies in Suffolk County. The role involves crisis intervention, group facilitation, and ongoing case management related to housing, transportation, and other needs.
