Family Support Partner: Care Coordination & Peer Mentoring
Comprehensive Community Action Program
Comprehensive Community Action Program in Rhode Island seeks a Family Support Partner to provide peer mentoring and care coordination to children, adolescents and families. This full-time role supports families and coordinates services across programs. You will conduct assessments, develop service plans, and deliver home visits while assisting with daily living skills and access to benefits.
