Pharmacist I – Sharp Memorial Hospital – Variable Shift – Per Diem Sharp HealthCare Hours Shift Start Time: Variable Shift End Time: Variable AWS Hours Requirement: 10/40 - 10 Hour Shift Additional Shift Information: Weekend Requirements: As Needed On-Call Required: No Hourly Pay Range (Minimum - Midpoint - Maximum) $79,510 - $102,600 - $114,910 The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. What You Will Do Provide professional, clinical and technical knowledge and skills required for comprehensive pharmaceutical care that supports the mission, values and philosophy of Pharmacy Services and Sharp HealthCare. Required Qualifications Other: Education pursuant to licensure requirement. California Pharmacist License - CA State Board of Pharmacy - REQUIRED ACLS Certification (Advanced Cardiac Life Support) - American Heart Association - REQUIRED AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association - REQUIRED Preferred Qualifications 1 Year experience in hospital pharmacy practice. 1 Year completion of PGY1 Pharmacy Residency. Other Qualification Requirements AHA BLS and ACLS required within 90 days of hire. Essential Functions Verification and review of medication orders: Reviews all medication orders for appropriate dose, duration, frequency, dosage form, and indication. Monitors therapy for potential or actual drug interactions (e.g., drug‑drug, drug‑disease, drug‑allergy, etc.). Ensures compliance with inpatient drug formulary and provides patient counseling as applicable. Education modalities include first dose teaching, food‑drug interaction counseling, discharge counseling, medication classes, and compliance aids. Medication reconciliation: Participates in admission and discharge reconciliation at sites where pharmacists or technicians are involved, including documentation of activities as needed. Medication Therapy Monitoring: Ensures drug regimens are consistent with Sharp guidelines and policies. Adjusts regimens based on protocols and interdisciplinary consultation. Conducts daily profile reviews for each patient, including age, sex, height, weight, renal/hepatic function, pregnancy status, dosage appropriateness, laboratory values, and microbiology results. Clinical Intervention Program: Qualitative participation through issue identification, best practice recommendations, and documentation. Communicates therapeutic issues to clinicians in a timely manner and follows up on formulary compliance. Pharmacy Operations: Prepares and dispenses medications accurately and timely. Maintains controlled substance accuracy, record integrity, and room inventories. Utilizes technology such as EMR, Pyxis, DoseEdge, and IV workflow systems. Demonstrates competence in IV admixture, aseptic technique, and chemotherapy preparation. Participates in code‑blue emergencies as required. Safe Medication Practice, Regulatory, and Quality Improvement: Participates in medication safety event monitoring, data collection for MUEs, inspections, and quality improvement initiatives. Ensures compliance with medication storage standards and regulatory requirements. Communication and Teamwork: Provides customer service to external and internal customers, answers phone professionally, fosters positive relationships, and ensures clear handoffs between shifts. Addresses complex situations through policy knowledge and escalation protocols. Operational Services: Intercepts and troubleshoots problems, prioritizes issues, evaluates workflow, and directs daily workload to maximize efficiency. Training and Mentoring: Orients and trains new staff, provides direction to pharmacist residents and interns, and engages in professional development. Knowledge, Skills, And Abilities Proficiency in computer skills, especially word processing. Pharmaceutical knowledge and skills. Ability to learn computer applications applicable to the role.
