Job Responsibilities

• Evaluate, diagnose,and treat patients with physical impairments in a SkilledNursing Facility setting
• Develop and implementindividualized treatment plans based on patient needs andclinical findings
• Supervise and collaborate with PhysicalTherapist Assistants to ensure effective care delivery
• Monitor patient progress and modify treatment plans asneeded
• Educate patients and caregivers on mobility,safety, and home exercise programs
• Maintain accurate andtimely documentation in compliance with facility and regulatorystandards

Requirements

• Active state license as a Physical Therapist in California
• Current BLS/CPR certification
• Minimum of 1 year of recent PTexperience preferred, ideally in a SNF or geriatric setting
• Strong clinical reasoning and leadership skills
• Ability towork collaboratively within an interdisciplinary team
