Field Care Manager, Behavioral Health

Humana is seeking a Field Care Manager, Behavioral Health to coordinate and deliver integrated behavioral health support to members in the community. In this role, you will conduct clinical assessments, create person-centered care plans, and collaborate with physicians, social workers, and community resources to address mental health, substance use, and social determinants of health. You will monitor progress, advocate for needed services, support care transitions, and provide crisis intervention as needed.
