Registered Nurse Case Manager

In this role, you will complete clinical assessments, create and manage individualized care plans, and support safe transitions between inpatient, outpatient, and home settings. You will collaborate closely with physicians, pharmacists, and interdisciplinary teams to close care gaps, reduce avoidable hospitalizations, and improve quality metrics.
