Travel Physical Therapist Skilled Nursing Facility

Schedule: Sunday - Thursday or Tuesday - Saturday or Monday - Friday

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 38 per week

Requirements:

• Active CA PT License
• SNF Experience Preferred (Mentorship Available for 0-1 Years Experience)
• Strong Documentation and Time-Management Skills

Roles & Responsibilities:

• Evaluate and treat patients in a skilled nursing facility.
• Develop and implement individualized treatment plans to improve mobility, strength, and overall functional independence.
• Collaborate with the interdisciplinary care team to enhance patient outcomes.
• Complete documentation in Net Health per facility standards.
