Bilingual Community Health Advocate

Oak Street Health is seeking a Community Health Worker to bridge patients with community resources and medical systems. A bilingual Spanish speaker will help promote health literacy and access to services while serving as a patient advocate in diverse communities. The CHW will work with Medical Social Workers, coordinate care plans, and meet patients in centers, homes, or community sites to support health outcomes.
