Covers 3 locations per day -- approx. 6 locations per week
Panel of 340 patients across 8 facilities
Patient population: ages 55 and older
Mix of short-term and long-term residents
Chronic disease management required -- HTN, DM, CHF, COPD, CKD
GI management required -- GERD, constipation, malnutrition, incontinence
Experience in geriatric post-acute care required
Hospital privileges required
