Hospice Registered Nurse Case Manager

Addus Family Company seeks a Hospice Registered Nurse Case Manager to provide compassionate end-of-life care in patients’ homes. You will manage a caseload of hospice patients, performing comprehensive assessments, developing and updating individualized care plans, and delivering skilled nursing care focused on pain and symptom management. The RN coordinates with physicians, social workers, chaplains, and aides, ensuring regulatory compliance and high-quality documentation. You’ll educate and support patients and families, participate in on-call rotation, and embody our mission-driven culture of compassion, respect, and integrity.
