In this Home Health RN role, you will provide direct, one-on-one nursing care to patients in their homes throughout Gainesville and surrounding areas. Your workday typically includes performing skilled nursing visits, conducting comprehensive assessments, implementing and updating personalized care plans, and coordinating with physicians and the broader care team. You will monitor vital signs, manage medications, provide wound care and post-surgical care as appropriate, and educate patients and families on disease management and safety in the home. A typical caseload involves adult and geriatric patients with a variety of diagnoses, including chronic conditions such as heart failure, COPD, diabetes, and post-orthopedic or cardiac surgery recovery. Visit volumes are structured to support thorough, high-quality care and adequate documentation time. You can expect a mix of routine visits, start-of-care admissions, recertifications, and discharge planning. Patient ratios are designed to allow focused, individualized attention at each visit rather than high-volume bedside assignments. You will work closely with an interdisciplinary team that may include physical, occupational, and speech therapists, medical social workers, and home health aides. Collaboration is an important part of the role, as you will communicate changes in condition, update care plans, and participate in case conferences to ensure comprehensive, coordinated care. The home health environment also offers opportunities to specialize your interests over time, whether in wound care, chronic disease management, or palliative-focused support. Shifts generally follow daytime hours, with scheduling that may include some rotation for on-call or weekend coverage depending on agency needs.
