Mental Health Worker Outpatient Adult Behavioral Health (ABH) Urgent Care

We are seeking a dedicated and compassionate Mental Health Worker to support our full-time case manager in a busy Adult Behavioral Health Urgent Care Clinic in Philadelphia, PA. This individual will assist with aftercare planning, follow-up calls, and discharges for clients visiting our walk-in mental health clinic.

Key Responsibilities:

• Assist with discharge coordination and aftercare planning for mental health clients.
• Conduct follow-up phone calls to ensure clients are connected to appropriate outpatient and community services.
• Support clients in navigating mental health recovery plans and treatment goals.
• Participate in treatment team discussions and contribute to individualized care plans.
• Provide direct client support and ensure the safety and well-being of individuals in the clinic setting.
• Document client interactions and updates in a timely and accurate manner.

Requirements:

• Bachelor's degree required.
• Minimum of 3 years of experience working in mental/behavioral health, social services, or a related field.
• Valid driver's license required.
• Experience with mental health clients preferred.
• Strong communication and documentation skills.
• Ability to work well in a fast-paced, team-oriented environment.

Preferred Skills:

• Knowledge of mental health diagnoses, treatment plans, and community resources.
• Experience with crisis de-escalation and client support.

Note: This is a short-term 8-week contract position designed to support urgent behavioral health needs in a high-demand walk-in clinic setting.
