Sign-On Bonus: $20,000 Available

Position: Home Health Physical Therapist

In this role, your responsibilities will include:

• Planning and delivering prescribed physical therapy treatments to patients in their homes, aiming for the highest possible level of physical function.
• Assessing and documenting patients' physical capabilities to assist in physician evaluations and inform treatment objectives.
• Creating and executing rehabilitation programs that align with the physician's treatment plan and the goals set by the patient care team.
• Providing clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, while supervising their performance and documentation.
• Thoroughly documenting patient observations, interventions, and evaluations in a timely manner.
• Communicating patient progress to the physician, Clinical Manager, care team, patients, and their families, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.

• Degree from an accredited Physical Therapy Program recognized by the APTA.
• Current and unrestricted Physical Therapy license.
• Valid CPR certification.
• Experience working collaboratively with a team of healthcare professionals.

Preferred Qualifications:

• At least one year of experience as a Physical Therapist.

Compensation Details

The pay structure for this position includes:

• $59.00 - $83.00 per visit/unit.
• Annual base salary ranging from $93,000 to $128,000.

Additional InformationTuberculosis Screening:

This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. Candidates selected for this role will be required to undergo TB screening.

Driving Requirements:

This role is also part of our driver safety program, necessitating a valid state driver’s license and personal vehicle liability insurance. Individuals must maintain vehicle insurance that meets or exceeds their state’s minimum requirements, or at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is higher.
