About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 8 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS within the last 6 months Description: Provides coordination and direction of home nursing care according to physicians' orders. Responsible for the competent application of the nursing process, adhering to agency policies and procedures. Supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Handles an average of 5-10 patients per day. Each visit and activity holds a different point value for productivity expectations. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Expectation to accept every patient on the schedule regardless of distance from previous appointments. All documentation must be completed within 24 hours of the appointment with the patient. There are no guaranteed hours with this contract, and clinicians must meet weekly productivity points. Clinicians may need to travel up to 75 miles from their assigned care center. All travelers must be proficient in HomeCare HomeBase and OASIS, or be willing to complete a retraining course if not used within the last 6 months. Local candidates are accepted at the same rate. No pending licenses are accepted. Travelers must be prepared for potential on-call rotations.
