Registered Nurse (RN) Team Lead - Skilled Nursing Facility

Job Summary

Join a dedicated healthcare team at a 120-bed Skilled Nursing Facility in Montour Falls, New York. This facility is structured into three units, each accommodating up to 40 residents, and is committed to delivering high-quality, community-focused patient care.

This role is designed for a Professional RN Team Lead who will provide direct patient care, including assessment, monitoring, recording, and reporting patient conditions. The successful candidate will collaborate with interdisciplinary teams, advocate for patients, and demonstrate strong leadership in time management, critical thinking, delegation, and prioritization. Flexibility is key, as the role may require functioning in an LPN capacity based on staffing needs, with potential for floating to other units.

Key Responsibilities

• Provide direct patient care, including comprehensive assessment, monitoring, recording, and reporting of patient conditions and changes.
• Demonstrate exceptional time management, critical thinking, delegation, and prioritization skills in a fast-paced environment.
• Collaborate effectively with the interdisciplinary care team to ensure coordinated and holistic patient care.
• Act as a patient advocate, ensuring their needs and preferences are respected and addressed.
• Practice ethically and professionally, adhering strictly to the scope of practice and facility policies.
• Promote and integrate Evidence-Based Practice (EBP) and best practices into daily patient care.
• Maintain clinical competence through continuous learning and completion of all required organizational education within specified timeframes.
• Document patient care accurately, thoroughly, and timely in the Electronic Medical Record (EMR).
• Actively participate in unit councils and staff meetings to contribute to unit improvements and team cohesion.
• Be prepared for physical demands including lifting up to 50 lbs and prolonged periods of sitting or standing.
• Operate all required medical equipment safely and proficiently.
• Demonstrate flexibility by floating to other units as staffing permits and needs arise.
• Be prepared to function in a Licensed Practical Nurse (LPN) capacity, including Med Cart duties, as staffing needs dictate.

Required Qualifications

• Ability to function effectively in both Registered Nurse (RN) and Licensed Practical Nurse (LPN) capacities based on staffing requirements.
• Successful completion of a 1-day unit-specific orientation.
