The Physical Therapist develops individualized treatment plans, monitors patient progress, and collaborates with interdisciplinary healthcare teams to support optimal patient outcomes.### Key Responsibilities* Conduct comprehensive physical therapy evaluations and assessments.* Develop individualized treatment plans based on patient needs, goals, and clinical findings.* Implement therapeutic interventions including manual therapy, exercise programs, and functional mobility training.* Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes.* Educate patients and families on treatment plans, home exercise programs, injury prevention, and wellness strategies.* Maintain accurate and timely patient documentation in accordance with regulatory and organizational standards.* Collaborate with physicians and other healthcare professionals to coordinate patient care.* Ensure compliance with all applicable healthcare regulations, safety standards, and ethical guidelines.### Qualifications* Doctor of Physical Therapy (DPT) or equivalent degree from an accredited physical therapy program.* Active state Physical Therapist license in the state of employment.* Current CPR/BLS certification.### Preferred Skills and Competencies* Strong clinical assessment and problem-solving skills.* Excellent communication and patient education abilities.* Ability to develop individualized treatment plans and track progress effectively.* Strong interpersonal skills and ability to work collaboratively in a multidisciplinary healthcare environment.* Commitment to providing high-quality, patient-centered care. #J-18808-Ljbffr
