Discharge & Care Coordination Case Manager
Cooper University Health Care
Cooper Norcross Health in Camden, NJ seeks a qualified social worker to provide psychosocial assessments, crisis intervention, and discharge planning for patients and families.
The role requires a master's degree in social work or a licensed RN, with NJ licensure preferred; ACM/CCMC credentials are also preferred. You will coordinate referrals, insurer authorizations, and inpatient hospitalization details with the Utilization Management team.
