Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating and developing treatment plans for assigned patients in accordance with their needs and physician direction. This role emphasizes the delegation and direction of discipline-specific therapy services, ensuring functional treatment programs are designed to facilitate a return to community activities. The Physical Therapist will also educate patients and families while adhering to the organization's core values and compliance expectations.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop and implement appropriate treatment plans and goals focusing on patient progress towards discharge.
• Maintain clear and concise medical record documentation, including goal writing and functional quality indicator assessments.
• Ensure care is provided under valid physician orders and within insurance approval parameters.
• Participate in student programs as assigned, providing supervision and developmental feedback.
• Engage in continuous professional development and education activities.
• Comply with State Practice Act and maintain professional licensure in good standing.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license to practice as a Physical Therapist.
• Prior experience with Select Medical or similar agency is highly desired.

Schedule

This is a 5x8 schedule with evaluations lasting 60 minutes followed by follow-ups every 30 minutes, starting in July.

Assignment Duration

This is a 13-week contract position.
