Geriatric RN Case Manager — Weekdays in Home-Based Care

WelbeHealth is seeking a Case Manager, RN to guide participant care within the IDT, conducting assessments and developing care plans for frail adults in a value-based program. You will work Monday through Friday with no weekends or on-call, coordinate with the PACE physician, and support transitions of care while delivering nursing interventions and education to participants and families. #J-18808-Ljbffr WelbeHealth
