Location: Cupertino, CA

About the Position

• Assist Occupational Therapists in implementing individualized treatment plans
• Provide occupational therapy services in an SNF setting
• Help patients improve ADLs, coordination, balance, and mobility
• Monitor patient progress and communicate updates to supervising OT
• Maintain accurate and timely documentation in compliance with facility standards
• Educate patients and caregivers regarding therapy exercises and recovery goals
• Ensure high standards of patient care and professionalism

Requirements

• Active California COTA License required
• Previous SNF experience preferred, but not required
• Strong communication and interpersonal skills
• Ability to work independently and within a team environment
• Commitment to quality patient care and positive outcomes
