Home Health/Hospice Registered Nurse (RN)

The Home Health/Hospice Registered Nurse (RN) provides comprehensive nursing care to patients in a home setting. The primary responsibilities include assessing patient needs, developing and implementing care plans, and coordinating patient care while ensuring that the highest quality standards of practice are adhered to.

Key Responsibilities

• Conduct initial assessments of clients and their families to determine home health needs.
• Perform thorough physical assessments and gather medical history regarding current and past illnesses.
• Regularly re-evaluate clients' nursing needs and make necessary adjustments to care plans.
• Develop individualized care plans based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative nursing actions.
• Engage clients and families in the planning process for their care.
• Administer medications and treatments as prescribed by the physician.
• Provide health care instructions to clients as per the assessment and care plan.
• Identify discharge planning needs during the care plan development and implement them prior to discharge.
• Act as Case Manager when assigned and coordinate care for an assigned caseload.
• Prepare clinical notes and update the primary physician as necessary, at least every sixty days.
• Communicate with physicians regarding any changes in client conditions and obtain necessary orders.
• Ensure arrangements for equipment and other necessary items and services are available for client care.
• Instruct and supervise home health aide care provided to clients.

Patient Demographics

This position involves providing care to a diverse patient population, including adults, geriatrics, and pediatric patients, across various settings such as homes and nursing facilities.

Qualifications

• Current BLS certification is required.
• Valid Driver's License and auto insurance are necessary.
• Previous experience in Home Health and Hospice care is required.
• Must adhere to radius guidelines for patient care.

Work Schedule

Shifts are from 8 AM to 5 PM, with an on-call rotation required for evenings, weekends, and holidays.

Scrub Attire

Any solid color scrubs are acceptable.

The electronic medical record system used for this position is HomeCare HomeBase.

Cancellation Policy

The organization reserves the right to cancel up to 4 shifts during a 13-week assignment, or 3 shifts for assignments that are less than 13 weeks.
