Senior Social Services Director

SOCIAL SERVICES DIRECTOR, Generations Applewood, Matteson, IL Generations at Applewood, in Matteson, IL, has an Opening for a Social Services Director with experience in a Skilled Nursing setting. This is a Full Time Position with Full Benefits, Pleasant Working Conditions, and the Opportunity for Advancement. Salary is $60,000 - $65,000 annually. Generations at Applewood is a beautiful 24-hour rehabilitation and skilled nursing community offering quality healthcare services to senior residents throughout the Chicagoland area. Learn more about our facility at? We seek a compassionate, experienced individual with 2-4 years experience and a Bachelor's Degree in Social Work (BSW or human services field. Master's degree preferred. You will foster a climate that enables our wonderful residents to maximize their individuality, independence and dignity. Our goal is to provide our residents with the highest practical level of physical, mental and psychosocial well-being and quality of life. Responsibilities of The Social Services Director: Conducts initial psychosocial evaluations upon admission Updates Minimum Data Set (MDS) Documentation within the Assessment Reference date Builds interdisciplinary care plans Safeguards resident rights Connects residents with financial or community resources (Medicaid and Social Security) Assist with planning, developing and directing our facility’s social service programs in accordance with current federal, state and local standards, guidelines and regulations Ensure that medical, emotional, and social needs of each resident is met/maintained Greet and orient new residents Direct hospice and palliative care Coordinate discharge planning Facilitate regular care plan meetings with residents and their families Address residents' or family grievances and assist nursing or other pertinent departments in resolving issues Attend/facilitate intervention, rehabilitation, marketing, performance improvement and Medicare meetings as appropriate Requirements of the Social Services Director: BA or Master’s Degree in Social Work (MSW) - or - BA in Psychology or Sociology A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) desirable Minimum two years' supervisory experience A minimum of 2-4 years of experience in field of social services in a hospital, nursing or long-term care facility Demonstrated knowledge of the social dynamic of the geriatric/LTC population Knowledge of community resources Experience with MDS and the use of Point Click Care Initiative to seek new methods and services for residents Strong leadership and relationship building skills High level of organizational professionalism Working knowledge of government regulations as relevant to social services “Generations - Our Family Caring for Your Family for Generations to Come” Equal Opportunity Employer #J-18808-Ljbffr
