Registered Nurse (RN) - Home Health Case Management

Position Overview Seeking a compassionate RN Home Health Case Manager to oversee patient care, collaborate with interdisciplinary teams, and ensure regulatory compliance. Key Responsibilities Manage a personal caseload, conduct routine visits, evaluations, and start-of-care assessments. Educate patients on health conditions such as CHF, COPD, Diabetes, and Wound Care. Collaborate with physicians to obtain orders and develop diagnosis-driven plans of care. Work alongside therapy teams including PT, OT, and Speech. Supervise LPN visits and organize your schedule to ensure efficient care delivery. Qualifications & Skills Active Missouri or Compact RN license, valid Missouri Driver's License, and preferably home health experience. Strong clinical assessment skills, organized, self-directed, and compassionate communicator with patients, families, and physicians.
