Nonprofit Case Manager

Naples, Florida | Full-Time | $50,000 - $65,000

What You'll Do

As a member of the Support Services Team, you will serve as an advocate, coach, and resource for Veterans participating in our housing programs. You will help Veterans identify barriers, develop personalized action plans, connect with community resources, and celebrate milestones as they move toward self-sufficiency.

Responsibilities include:

• Conducting comprehensive needs assessments
• Developing individualized case plans
• Meeting regularly with Veterans to monitor progress and provide encouragement
• Connecting clients with medical, mental health, employment, housing, transportation, education, legal, and community resources
• Coordinating services with internal staff and community partners
• Maintaining accurate client documentation, case notes, and electronic records
• Providing crisis intervention when needed
• Preparing reports and maintaining organized client files
• Participating in community partnerships and provider meetings
• Supporting shelter orientations and assisting with additional program needs as needed

Qualifications include:

• Bachelor's degree in Human Services, Social Work, or a related field (BSW required; MSW preferred)
• Two to three years of case management or human services experience
• Experience working with Veterans, homelessness, mental health, substance use, domestic violence, or individuals experiencing poverty
• Excellent communication and relationship-building skills
• Strong organizational and time management abilities
• Professional judgment and the ability to establish healthy client boundaries
• Ability to work independently while collaborating with a supportive team
• Proficiency in Microsoft Office and electronic case management systems (HMIS experience is a plus)
• Valid Florida driver's license
• Ability to successfully pass a background check

• Bilingual (English/Spanish)
