RN - Data Entry Nurse

Good Samaritan is seeking a dedicated RN, Minimum Data Set (MDS) professional to support our mission of providing comprehensive, holistic healthcare services to underserved communities. As the RN MDS, you will be responsible for coordinating and completing accurate and timely Minimum Data Set (MDS) assessments, ensuring regulatory compliance, and translating patient assessments into individualized, person-centered care plans that support meaningful health and wellness outcomes. Working within our Healthcare & Medical Services department, you will collaborate closely with an interdisciplinary team—including physicians, nurses, therapists, social workers, and care coordinators—to gather clinical information, validate documentation, and ensure that each resident’s needs are reflected in the overall plan of care. In this role, you will perform comprehensive clinical assessments, review medical records, and enter MDS data into the electronic health record (EHR) system in alignment with CMS and state‑specific regulations. You will participate in care plan meetings, contribute nursing expertise to support functional improvement and quality of life, and help drive outcomes that reflect Good Samaritan’s commitment to compassion, integrity, and community engagement. The RN MDS will also support quality improvement initiatives by analyzing assessment data, identifying trends, and partnering with leadership to address gaps in care, documentation, or compliance. You will assist with preparation for regulatory surveys and audits, ensuring documentation supports the clinical acuity and services provided.
