This role involves providing patient-centered care either in home settings or hospice facilities, ensuring comfort, dignity, and quality of life for patients and their families throughout the hospice journey.Key ResponsibilitiesPerform comprehensive assessments addressing physical, emotional, and spiritual needs of patients.Conduct initial physical evaluations and develop individualized care plans in collaboration with a multidisciplinary healthcare team.Administer medications and manage symptoms following established protocols.Educate and support patients and families about end-of-life care options and resources.Coordinate services to facilitate smooth hospice care delivery.Maintain accurate and timely documentation of patient care activities and communicate changes in patient condition to the interdisciplinary group.Perform venipuncture or collect lab specimens as ordered by physicians.Ensure communication with primary and consulting providers regarding patient status.Education and ExperienceGraduate of an accredited nursing program with current Registered Nurse licensure valid for practice in the region.Previous experience in hospice, palliative, or critical care settings is preferred.Completion of hospice philosophy and care training is required.RequirementsCurrent Basic Life Support (BLS) certification according to American Heart Association standards.Completion or commitment to complete Mandatory Reporter training within six months of hire.Adherence to all federal, state, and professional licensure and regulatory training requirements.Strong oral and written communication skills.Basic computer proficiency and ability to perform simple mathematical calculations.This position offers part-time hours during evenings and nights, including PRN shifts, within a regional healthcare setting in the Midwest.
