MDS Coordinator RN

Fairview Skilled Nursing Home Position Summary Fairview Skilled Nursing Home is seeking an experienced MDS Coordinator RN to oversee the MDS/RAI process, ensure timely and accurate assessments, maintain regulatory compliance, and support quality resident care and reimbursement. Responsibilities Complete and coordinate MDS, CAAs, and care plans. Ensure compliance with CMS, Medicare, Medicaid, PDPM, and OBRA regulations. Monitor assessment schedules and submission deadlines. Collaborate with the interdisciplinary team on resident assessments and care planning. Review documentation for coding accuracy and reimbursement. Monitor Quality Measures and assist with QAPI initiatives. Support survey readiness and educate staff on MDS documentation. Qualifications Current New York State RN license. MDS Coordinator experience in a skilled nursing facility preferred. Knowledge of MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.
