Job Title Social Worker - Adolescent Health (Primary Care) Job Description The A28 Social Worker will provide social work services to at-risk adolescents and their families in an integrated medical and mental health primary care setting at the Mount Sinai Adolescent Health Center (MSAHC). Responsibilities Actively engage in assessment and treatment planning of patients referred from medical providers in Article 28 setting; provide short-term therapy as needed. Develop therapeutic alliance and deliver therapeutic interventions while consulting closely with medical providers and child and adolescent psychiatrists. Develop effective treatment plans and track treatment progress using validated instruments. Participate in interdisciplinary effort fostering a cohesive culture, improved communication, accountability, and integration of the MSAHC integrative model of care. Provide individual and family psychotherapy, intensive case management, crisis intervention, and health education to transgender and gender non-conforming youth. Provide assessments for gender dysphoria, hormone and surgery readiness as needed. Provide counseling related to medical transition. Ensure delivery within a youth-centric, high-quality, integrated collaborative culture that complies with Mount Sinai policies, New York State regulations, and funder expectations. Qualifications NYS LCSW or LMSW Commitment to MSAHC's mission, vision and approach to ensure a patient-centric culture that is collaborative and multidisciplinary Passion for working with young people with a work ethic prioritizing understanding and solving patient needs Demonstrated commitment to diversity, equity, inclusion and belonging Skillful in assessing patient needs related to health and psychosocial functioning and establishing interventions accordingly Significant experience working with low-income, inner-city, HIV positive, at-risk adolescents Experience delivering therapeutic interventions such as preventative and risk-reduction models that reflect developmentally appropriate methods and materials in working with adolescents and young adults (e.g., TF-CBT, CBT, MI, DBT, EMDR, CPT, IPT-A) Experience providing individual therapy and group work Experience working with populations with complex trauma histories (preferred) Experience working with LGBTQIA identified adolescents and young adults (preferred) Ability to be flexible and use nontraditional approaches in working with young adults (preferred) Excellent collaborative, clinical and communication skills Excellent organizational and documentation skills Skillful in collaborating with external resources and coordinating appropriate referrals Working knowledge of state regulations related to child abuse, neglect, and other urgent care issues Bilingual (English/Spanish) (preferred) Equal Opportunity Employer The Mount Sinai Health System is an equal opportunity employer, complying with all applicable federal civil rights laws. Compensation Mount Sinai Health System (MSHS) provides a salary range of $88,691 to $101,671 annually. Actual salaries depend on experience, education, and hospital need.
