Full-Time | Monday–Friday, 8:30 a.m.–5:00 p.m.
Salary: $100,000–$108,000 per year

BAYADA HOME HEALTH CARE is seeking a Registered Nurse (RN) Clinical Manager for our Long Island Nassau Assistive Care office in Garden City, NY. This office provides compassionate, high-quality personal care and support to clients throughout Nassau County, helping them remain safely and comfortably at home.

As a Clinical Manager, you’ll lead and support field staff, provide clinical oversight, strengthen client relationships, and help ensure consistent, high-quality care delivery throughout the community. This is an office-based leadership role with regular field responsibilities throughout Nassau County; it is not a remote position.

Responsibilities for a Clinical Manager

• Lead, inspire, and support Home Health Aides and other field staff in delivering exceptional care.

• Visit clients throughout Nassau County and conduct assessments, reassessments, supervisory visits, and routine client check-ins.

• Develop individualized care plans and oversee their implementation.

• Monitor client needs, service delivery, and changes in condition.

• Supervise, mentor, and coach caregivers in the field.

• Facilitate orientation for newly hired staff.

• Conduct competency assessments for new and tenured employees.

• Partner with families, caregivers, referral sources, and office leadership to support continuity of care.

• Provide office phone coverage and assist with lunch coverage as needed.

• Support sister offices within the Long Island region, including Suffolk, on an interim or as-needed basis.

• Respond flexibly to occasional last-minute or unplanned visit needs.

• Participate in occasional weekend or evening visits when needed; these needs are rare.

• Represent BAYADA at community marketing events and health presentations.

• Promote compliance with BAYADA policies, applicable regulations, and documentation standards.

• Participate in quality improvement initiatives and help resolve client or employee concerns.

• Model The BAYADA Way through compassionate leadership, clear communication, approachability, and dependable follow-through.

Qualifications for a Clinical Manager

• Current Registered Nurse (RN) license in good standing in New York.

• Minimum of two years of RN experience.

• Prior supervisory or leadership experience preferred.

• Home care, personal care, or community-based healthcare experience preferred; strong hospital or bedside nursing experience, including ICU experience, may also be considered.

• Outgoing, empathetic, approachable, and relationship-oriented communication style.

• Demonstrated ability to collaborate with colleagues and support the needs of the broader team and sister offices.

• Comfort working in varied client home environments, including homes with pets or tobacco smoke.

• Strong clinical assessment, organization, communication, and problem-solving skills.

• Ability to build trusting relationships with clients, families, caregivers, and colleagues.

• Passion for supporting clients’ independence, dignity, and quality of life at home.

• Ability to travel throughout Nassau County for client visits and community engagement.

• Flexibility to manage changing priorities, occasional last-minute requests, and a growing caseload.

Day-to-Day Caseload and Schedule

• Work from the Garden City office Monday through Friday, 8:30 a.m.–5:00 p.

• Balance office-based clinical leadership, caregiver support, client visits, phone coverage, and community engagement.
