About the Position

Job Duties: The RN Care Coordinator is responsible for determining the appropriateness of hospital admission, and for advocating, coordinating and facilitating the interdisciplinary plan of care to expedite medically appropriate, effective, efficient, and time utilization of resource for maximum patient outcomes.

Requirements

Must have 2 years of acute care Case Management Experience. See Special Procedures/Unit Details. Traveler MUST HAVE strong EPIC experience and strong acute care experience

Case Manager - 4/10s Days

Need to work 2 weekend shifts per 4 week period
