Claratel Behavioral Health is seeking Project HEAL Case Managers to support individuals transitioning from hospital and acute care into Transitional Housing. You will coordinate safe, clinically informed transitions, ensuring continuity of medical, behavioral health, and substance use treatment services. You will provide engagement, screening, care coordination, and linkage to ongoing treatment through Claratel Behavioral Health and community resources, helping clients achieve stabilization, #J-18808-Ljbffr Claratel Behavioral Health
