The registered nurse (RN) primary care patient aligned care team (PACT) coordinator will demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. Serves in an administrative role as a PACT champion and coordinates local PACT implementation, training, communication, and sustainability of the PACT model. Actively promotes the PACT model within the ambulatory care setting, educates core PACT team members.
