Foster Care Case Manager

Foster Care Case Manager $50,000 Do you have experience in working with individuals with developmental disabilities or mental health issues? Bring your clinical skills to a team‑based workplace that puts people first. As the Clinical Coordinator, you will advance the company’s mission of providing quality service by facilitating and providing case management and counseling services. Provide case management and counseling services to an assigned individual caseload. Assess referred individuals and lead development, coordination, and evaluation of service plan in conjunction with treatment team, funding source, and key stakeholders. Maintain current and complete records in accordance with corporate, contractual, and regulatory standards. Provide ongoing monitoring and consultation to mentors regarding implementation of service plan, including pre‑service introduction and continuing education. For caseloads that include mentor individuals, make mentor and family home visits, and conduct mentor group meetings as required. Assist with emergency coverage and crisis intervention support as needed, including participation in rotational on‑call system. Assist with legal, licensing, regulatory and certification activities as needed. Participate in all activities designed to enhance and build an active network of mentors. Qualifications Bachelor’s degree in human services or related field. One year of work‑related experience working with individuals with intellectual and developmental disabilities, brain injury, youth placed at‑risk, etc. preferred. Current driver’s license, car registration, and auto insurance. Licensure and training as required by state such as CPR, First Aid, Behavioral Intervention Techniques, etc. A reliable, responsible attitude and a compassionate approach.
