Registered Nurse

Position Summary Buffalo Homecare is seeking a dedicated and experienced Registered Nurse (RN) to join our team in Buffalo, NY. This full-time position is responsible for conducting client assessments, reassessments, aide supervisions, and nursing evaluations while ensuring the highest quality of care for individuals receiving home care services. This is an excellent opportunity for an experienced homecare nurse who is passionate about helping individuals remain safely and independently in their homes. Included with this position Company vehicle Company cell phone Company laptop Responsibilities Conduct comprehensive nursing assessments and evaluations. Complete new client assessments and periodic reassessments. Perform aide supervisory visits and competency evaluations. Collaborate with caregivers, physicians, service coordinators, and interdisciplinary team members to develop and implement individualized care plans. Educate clients and families on health management, safety, and self-care practices. Travel to client homes and community settings to provide nursing services and evaluations. Participate in interdisciplinary meetings and care conferences. Maintain accurate and timely documentation in accordance with agency, state, and federal requirements. Ensure compliance with all healthcare regulations, policies, and professional standards. Participate in ongoing training and professional development opportunities. Qualifications Current and unrestricted Registered Nurse (RN) license in New York State. Minimum of 3 years of nursing experience required. Minimum of 1 year of home care experience required. Strong clinical assessment and care coordination skills. Excellent communication, organizational, and interpersonal skills. Ability to work independently and manage a flexible schedule. Experience with Electronic Medical Records (EMR) systems preferred. Valid New York State driver's license required. Basic Life Support (BLS) certification required.
