Cadence is hiring a bilingual Spanish-speaking Clinical Navigator (Health Coach) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. Clinical Navigators deliver proactive, patient-centered outreach and follow-through, including coaching and care coordination, with documentation in the Cadence platform and collaboration with the clinical team to escalate as needed. #J-18808-Ljbffr
