We are seeking an Occupational Therapists (OT) to join supportive, patient-focused therapy team in a skilled nursing facility .

Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy services to help patients improve mobility, strength, balance, and independence
- Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
- Document patient progress and adjust treatment plans as appropriate
- Provide patient and caregiver education
- Support safe discharge planning and continuity of care
Qualifications
- Current California Occuaptional Therapist license
- Graduate of an accredited Occupational Therapy program
- Strong communication and patient-care skills
- Multiple Sacramento-Area Locations
