Schedule: This position follows a full time schedule with hours varying. Flexibility may be required based on patient and operational needs.

Shift Differentials: This position may be applicable for shift differentials based on the current policy at the time of hours worked.

Essential Duties and Responsibilities:

• Complete an Assessments tool with patients that arrived at our ED, and or Ambulatory Clinics
• Assist with community resources and scheduling appointments
• Facilitate referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment, etc.)
• Refer to primary care practices for ongoing follow up and engagement with a primary care provider
• Support and closely coordinate behavioral health care with the patient's PCP and, when appropriate, mental health providers
• Encourage patients to become actively engaged in their own health through calm, compassionate communication and by utilizing techniques such as motivational interviewing and behavioral activation
• Accurately document patient information, assessments, interventions and encounters
• Attend ongoing training and learning related to job position and duties
• Proactively outreach to patients to encourage them become actively engaged in their own health through calm, compassionate communication and by utilizing techniques such as motivational interviewing and behavioral activation
• Serve as a resource for participants in the program
• Develop referral relationships with healthcare providers, social workers and other community based service providers
• Schedule patient appointments
• Strong organizational and planning skills
• Provides outreach to the community
• Assist in the development of resource materials and preparation and maintenance of accurate records and data
• Complete all reports to document activities and provide data to measure impact
• Performs other administrative office duties
• Maintains department orderliness and cleanliness
• Other duties as assigned
