Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Illinois lic. required.
• EMR: Cedaron
• Must have a verifiable permanent address 50 miles or more from facility.

Assignment Details

• Schedule options include 4x10s or 5x8s.
• No weekends.
