Job Title ACT Nurse/Nurse Location Hermiston, OR FLSA 1.0 FTE Non-Exempt (Expectation to work 40 hours per week) Mon-Fri 8am-5pm Supervisor Clinical Supervisor Pay Grade N14 ($53.94 - $65.29, depending on experience) Additional Compensation Potential for 10% bilingual increase – must pass the language test after hire. Hiring Bonus: $5,500 (2‑year commitment, staggered‑taxed payout). Benefits Health, dental and vision insurance 6% initial 401(k) match Potential for tuition reimbursement Paid vacation tiers ranging from 1 day to 4 days per month (annual rollover cap of 220 hours; additional hours can be paid out at 50% at fiscal year end) 9 paid holidays, community service day Floating holiday & 2 mental health days provided after 1 year introduction Workplace flexibility schedule options available after 1 year introduction (work from home hours vary by position & schedule) Relocation benefit up to $4,000 if moving over 100 miles (available to be included with job offer) Student loan forgiveness (NHSC & Public Service) Paid licensure supervision Employee assistance, wellness benefits, dependent care & long‑term disability insurance Description The ACT nurse, supervised by the ACT team leader/Clinical Supervisor, provides culturally competent and appropriate mental health nursing care to people served by Community Counseling Solutions (CCS) in the ACT program. The position provides treatment in accordance with the Oregon Administrative Rules (OAR’s) and the mission and values of CCS. The position works closely with other involved community partners and is professional in all aspects of service delivery. The nurse ACT will provide recovery, treatment, and support to clients, including medication oversight and mental health nursing care. Supervision Supervision received: Clinical Supervisor. Supervision exercised: none. Responsibilities Administers oral medications and injections following physician prescriptions and procedures. Takes and records measures of patients’ general physical condition such as pulse, temperature, and respiration to provide daily information. Leads prescribed individual, group therapy and/or education sessions as part of specific therapeutic procedures. Consults with community agencies and families to maintain coordination in the treatment process. Assists ACT participants in taking medications successfully independently using behavior‑tailoring techniques, coaching, reminders, and other strategies. Issues medications from dispensary and maintains records in accordance with specified procedures. Educates and trains staff on medication administration, ensuring documentation is kept according to policies. Participates in joint case planning and coordination with agency employees and relevant contractors; acts as liaison between clients and other agencies; advocates for clients when appropriate. Maintains client records using an electronic medical record; documents required interactions, behavioral health assessments, and individual service plans. Triage and assess clients for medical concerns and make referrals to hospital and other agencies. Formulates individualized nursing care plans and participates in patient care meetings. Consults with other health team members to provide guidance as necessary. Monitors patient condition, notes changes in status, utilizes judgment, and takes appropriate action. Completes all paperwork professionally, clinically soundly and timely as prescribed by OAR’s and other pertinent guidelines. Other duties as assigned. Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and Experience Requires sufficient education to qualify as a registered nurse; additional related experience or training is helpful. Must have a current registered nurse license issued by the state of Oregon. Must be able to pass a criminal history background check. Preference given to individuals who have worked with people with mental illnesses in a residential or psychiatric hospital setting. Other Skills and Abilities Knowledge of symptoms and challenges faced by people with mental illness and addictions. Knowledge of relevant OAR’s. Effective verbal and written communication; ability to work independently and complete responsibilities in a thorough and timely fashion. Professional skills to work with the public and coordinate services across agencies and community groups. Ability to develop cooperative and respectful relationships with clients and their families. Knowledge of the recovery model. Ability to represent the interest of the customer and the agency favorably in the community. Team‑working skills. Ability to handle highly confidential information and adhere to client confidentiality rules and laws. Functional knowledge of business English and medical terminology. Good spelling and basic mathematical skills. Ability to learn assigned tasks readily and adhere to general office procedures. Good organizational and time management skills. Knowledge of standard office equipment. Must communicate effectively in both written and oral formats, present and exchange information internally across teams and co‑workers, and externally with customers and the public. Must hold a valid driver’s license, be insurable or carry insurance for client service purposes, travel between business offices, community, and required meetings/trainings. Must pass all criminal history check requirements as stipulated by ORS 181.536-181.537 and OAR 410‑007‑0200 through 410‑007‑0380. Physical Demands Regularly use hands to finger, handle, smell, or feel; frequently stand, walk, stoop, kneel, crouch, or crawl; occasionally climb or balance; regularly lift and/or move up to 10 pounds; requires close and distance vision. Reasonable accommodations may be made for individuals with disabilities. Personal Auto Insurance Must hold a valid driver’s license and personal auto insurance for vehicles used for CCS business. Proof of $300,000 or more liability coverage for bodily injury and $100,000 or more in property damage is required during employment at CCS. The employee’s insurance is primary with CCS insurance being secondary. Work Environment Work is performed in a secure facility; may perform follow‑up duties in the community; noise levels usually moderate but may be loud. Occasional travel and overnight stays may be required for meetings and trainings. Some environments may be uneven terrain; safety precautions typical of office or home environments apply.
