Social Worker - MSW Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 2 years of acute hospital case management experience required License: CA state license preferred; pending license accepted Certifications: MSW Must-Have: 2 years of acute hospital case management experience Knowledge of drug/alcohol abuse treatment options Experience working with patients experiencing homelessness Familiarity with patients with mental health histories or in crisis Ability to organize and participate in family meetings Knowledge of discharge planning Description:The position involves providing social work services in an acute care setting.You will be expected to manage a caseload of up to 25 patients.Patient ratios during the day are approximately 1:35.The role includes focusing on discharge planning and care coordination.You will participate in the development of treatment plans.You must work weekends every other week and be available for holiday coverage.This role requires effective documentation according to the plan of care.You will educate patients and caregivers to facilitate discharge.Interdisciplinary support includes various healthcare teams and services.The ideal candidate should have strong communication and collaboration skills. #J-18808-Ljbffr
