Full-Time – Permanent or Travel | $75,000–$104,000/year
The OpportunityYou'll deliver skilled physical therapy across outpatient rehab, skilled nursing, and assisted living settings within a respected healthcare organization. You'll initiate and implement personalized physical therapy programs based on physician orders, working with geriatric, subacute, and adult patient populations. Your responsibilities include:
- Maintain detailed evaluations, progress notes, and discharge summaries to track functional outcomes
- Collaborate with nursing and rehab staff to enhance residents' recovery and independence
- Contribute to interdisciplinary care planning meetings to support holistic treatment approaches
- Track patient statistics, manage treatment documentation, and request necessary supplies
- Participate in or lead staff training and education sessions
- Provide leadership support in the absence of the Director of Physical Therapy
- Graduate of an accredited Physical Therapy program
- Active New York State Physical Therapy license (required)
- Strong interest in working with geriatric, subacute, and adult patient populations
- Leadership qualities and ability to direct support staff when needed
- Previous experience preferred, but new PT grads are encouraged to apply
