Home Health Weekend Admission Nurse

Pay: $145 per admission, mileage reimbursement and paid drive time

Schedule: Saturday and Sunday required, optional Friday and Monday

Join our compassionate care team as a Home Health Weekend Admission Nurse providing support throughout Salt Lake County. In this role, you'll admit patients, create care plans, and deliver meaningful, patient-centered care in the comfort of their homes.

What You'll Do:

Complete admissions and develop individualized Plans of Care
Provide skilled nursing visits for home health patients
Collaborate with case managers and interdisciplinary teams
Educate patients and families while supporting comfort and dignity

What We're Looking For:

Active Utah RN license
2+ years nursing experience (home health or hospice preferred)
Strong assessment, communication, and time management skills
Reliable transportation and CPR certification
