Experienced Dialysis LPN

Licensed Practical Nurse (LPN)Ditmas Park Dialysis Center is dedicated to providing high-quality renal care to our diverse patient population. Our goal is to deliver exceptional dialysis services using the latest technology and best practices in patient care.Position Summary:The Licensed Practical Nurse (LPN) will be responsible for providing direct patient care under the supervision of a Registered Nurse (RN) and in accordance with the policies and procedures of Ditmas Park Dialysis Center. The LPN will play a critical role in the daily operations of the dialysis unit, ensuring the safety and well-being of our patients while maintaining a high standard of care.Key Responsibilities:Monitor vital signs.Prepare patients for dialysis treatment, including cannulation of access sites.Administer medications and IV fluids as prescribed.Monitor patients during dialysis treatment, addressing any complications or concerns.Provide patient education on dialysis care, diet, and medications.Assist in the development and implementation of individualized patient care plans.Operate and maintain dialysis machines and equipment.Ensure the proper setup and breakdown of dialysis machines.Follow infection control and safety protocols to prevent cross-contamination.Conduct routine checks on equipment to ensure functionality and safety.Maintain accurate and detailed patient records.Document patient progress, treatment plans, and any changes in condition.Ensure all documentation complies with regulatory and organizational standards.Work closely with RNs, nephrologists, dietitians, and other healthcare professionals to provide comprehensive patient care.Participate in team meetings and contribute to continuous improvement efforts.Assist in training and mentoring new staff members.Participate in quality improvement initiatives to enhance patient care and operational efficiency.Report any incidents, errors, or safety concerns promptly to the Nurse Manager.Qualifications:Graduate of an accredited LPN program.Current and valid LPN license in the state of New York.Previous experience in a dialysis or acute care setting preferred.Strong clinical and assessment skills.Excellent communication and interpersonal skills.Ability to work effectively in a team-oriented environment.Proficiency in electronic health records (EHR) systems.Basic Life Support (BLS) certification required.IV Certification preferred.Physical Demands and Work Environment:Frequent standing, walking, bending, and reaching.Exposure to bloodborne pathogens and infectious diseases.Use of personal protective equipment (PPE) as required.Benefits:Competitive salary based on experience.Comprehensive health, dental, and vision insurance.Retirement savings plan with employer match.Paid time off and holidays.Continuing education and professional development opportunities.
