Key Responsibilities

• Conduct comprehensive patient assessments, taking into account medical history and current health status, while utilizing advanced diagnostic techniques.
• Develop and implement individualized patient care plans aimed at improving health outcomes, which may include medication management, lifestyle adjustments, and referral to specialists.
• Monitor patients for changes in condition, documenting all findings and communicating effectively with other healthcare team members.
• Order, interpret, and facilitate diagnostic tests, including but not limited to blood tests and imaging studies, ensuring that results are acted upon promptly.
• Administer treatments and therapies in accordance with evidence-based guidelines, ensuring adherence to all regulatory compliance requirements.
• Educate patients and their families on health maintenance, disease prevention, and treatment options, thereby promoting informed decision-making.
• Maintain meticulous patient records using our Electronic Health Record (EHR) system to ensure accurate and updated patient information.
• Actively participate in interdisciplinary team meetings to discuss ongoing cases and treatment plans, fostering collaboration and holistic care approaches.
• Adhere to all federal, state, and local regulations, clinical practice standards, and hospital policies to maintain the confidentiality and integrity of patient information.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse elderly population, many of whom require specialized care for chronic conditions. Qualifications

• Active Nurse Practitioner and Registered Nurse licensure in the state of Maryland.

• Minimum of 2 years of clinical practice as a Nurse Practitioner in a relevant healthcare setting.

Preferred Experience

• Experience in geriatric care or with chronic condition management is highly desirable.

Compensation and Benefits
