RN MDS Coordinator

At Lutheran Home in Arlington Heights, IL, we offer a salary range of $87,800-$99,700 based on experience for a full-time RN MDS Coordinator position. Your role will ensure every resident's assessment accurately reflects their physical, mental, and psychosocial status, directly determining the care plan and reimbursement for that care.

Responsibilities include:

• Coordinating the RAI process for accurate and timely resident assessments
• Ensuring MDS assessments reflect each resident's physical, mental, and psychosocial status
• Maintaining compliance with Medicare, Medicaid, OBRA, and other payer requirements
• Verifying documentation supports services provided and reimbursement accuracy
• Collaborating with the interdisciplinary team on care planning, assessment completion, and case management
• Tracking assessment schedules to meet regulatory timelines
• Supporting quality improvement work and survey readiness

Requirements include:

• Graduate of an accredited school of nursing with a current RN license in Illinois
• At least 2 years of MDS 3.0 and case management experience in a skilled nursing or post-acute setting
• Comfortable working independently while coordinating with an interdisciplinary team
• Working knowledge of Microsoft Office

Preferred qualifications include:

• Long-term care or skilled nursing experience
• Current RAC-CT certification, or willingness to obtain it within 6 months of hire and keep it current
